Requirements

Parties or their lawyers must provide our program with a copy of the court order which specifies the terms and conditions of parenting time (times, dates, and frequency of visits). Both parties are required to contact our program in order to book their respective intake interviews.

What do Residential Parties need to bring when attending the intake interview?

As the residential party (the parent or guardian with custody of the children) you are required to bring the following to your intake interview:

  • Your full address
  • Names and telephone numbers of two of your friends or relatives so we can contact them in case of an emergency
  • Your child’s picture
  • A picture of yourself
  • Your child’s physician’s or pediatrician’s name and phone number
  • Your child’s health card number
  • All assessments and affidavits with regards to custody and access

What do visiting parties need to bring when attending the intake interview?

As a visiting party you are required to bring the following to your intake interview:

  • Your full address

Who conducts the intake interview?

The Program Manager or Client Services Manager will conduct the intake interview.
